I was surfing KWWH while I see:-

Most users ever online was 5,458, 16th Oct 2010 at 06:31 PM.

This site has totall 12000 Users and 5500 online at one time>3

Is it true?

Users online include guests and bots and even a dos attack. Have you ever see the users online when a site is under a small dos attack? It's cool, it makes the site look really active. Other things that can affect users online is how long it's set to. I think its set to 5 minutes by default but that can be changed. Another thing is if a mysql process get's stuck it can back up everyone and loads of people are registered being online at the same time..... so ya it's not that accurate but it's still nice to see it's high and the traffic is growing.
